Eligibility is based upon contributions received on your behalf for hours worked for contributing employers under the jurisdiction of OFFICE & PROFESSIONAL EMPLOYEES INTERNATIONAL UNION LOCAL NO. 11. The rules for initial and continuing coverage are identical; that is, any time you work sufficient hours in one month, you will receive a month of coverage in another month. You and your dependents will become insured on the first day of the second month following a month in which you work the qualifying hours per your labor contract. The employer pays a contribution on your behalf (in the month following your work) on the basis of those qualifying hours. For example:
