If you suffer an accident or illness and your disability causes you to lose coverage (in a rolling 24-month period), you may be eligible for benefits for up to six months, at no cost to you, per a special Waiver of Premium provision of your Plan.  It is your responsibility to notify the Trust Office of your disability.  In addition, you may make self-payments to extend your coverage, according to the CONTINUATION OF COVERAGE (COBRA) section.
