  Loss of Eligibility
 

Employee:
If you fail to work the qualifying hours in any month, your employer is not obligated to make a contribution on your behalf and you will lose coverage at the end of the month following the month you failed to work sufficient hours.

For example: You worked steadily for many months and had continuous coverage, but failed to work enough hours in the month of March.  Your prior work months would provide you with coverage through April 30th, but because no payment would he made to the Trust in April for March work, there would be no coverage for the month of May.

Dependent:
Insurance ends for an enrolled child on the last day of the monthly period in which the child is no longer eligible according to the terms of the Plan.
